If your relative came to Ukraine and has been captured by the enemy side or is missing in action (or has suddenly stopped contacting you), please follow the steps below:

2. The form contains questions about your relative. Please fill in the answers in the form.
3. Send the completed form to:

   koord.sk3@gur.gov.ua

4. Coordination Headquarters' representatives will contact you within 3 business days. We will provide you with:
   1. All the available information about your relative (such as their current status in our database, possible place of detention in Russia, etc.). If we determine that your relative is neither missing nor captured.
   2. Consultation on the next steps in the process in the case if your relative is illegally detained, is missing or was killed.
