My relative joined of the Ukrainian Defense Forces unit and went missing or was captured. What do I do?
Note: If your relative was a civilian at the time they went missing or were illegally detained, please follow this guide for relatives of civilians.
If your relative joined service in one of the Ukrainian Defense Forces units (Defense Intelligence of Ukraine, Ukrainian Armed Forces, National Guard of Ukraine, Territorial Defense Forces, etc.) and has been captured by the enemy side or is missing in action (or has suddenly stopped contacting you), please follow the steps below:

- The form contains questions about your relative. Please fill in the answers in the form.
- Send the completed form to:
- Coordination Headquarters' representatives will contact you within 3 business days. We will work with you to confirm your immediate relation to the serviceman. Afterwards we will provide you with:
- All the available information about your relative (such as their current status in our database, possible place of detention in Russia, etc.). If we determine that your relative is neither missing nor captured (for example, they may have been wounded or their recent mission prevented them from contacting you).
- Legal consultation – regarding compensations and services that you are entitled to for free as a relative of missing, captured or killed in action serviceman of Ukrainian Defense Forces.
- Consultation on the next steps in the process in the case if your relative is in captivity, missing in action or was killed in action.
